- Also, as required by my ethics codes, please download the Informed Consent and bring in the last page, signed.
- If you are seeking services for a minor, please complete and bring the Parental/Gaurdian consent for the treatment of minors.
- Couples, please complete this one, online, each of you.
Questions about the forms? I will be happy to discuss them when we meet.
Why complete them before coming in? That way you are not sitting in the waiting area filling out forms while you are paying me for that time.
